Complete these forms and bring them to your first visit. Or if you’d prefer to fill them out at the office, just come 15 minutes before your scheduled appointment and we’ll have them for you.

All new patients must complete the new patient form.

If the primary reason for your visit is headaches, please also fill out the headaches form.

If the primary reason for your visit is neck pain, please also fill out the neck pain form.

If the primary reason for your visit is low back pain, please also fill out the back pain form.

Dr. Wicks will consult with you and explain our techniques and what’s expected of you as a patient. We’ll get a brief history from you so we can get a feel of what types of treatment you’ve had in the past, as well as any concerns or problems you’ve been having. Dr. Wicks will do his initial exam and start noting any areas to work on. We’ll then take the time to review with you all the specific areas that we will focus treatment on.

Many times you will receive treatment on your first visit. Further testing such as x-rays may be needed in some cases. Dr. Wicks will give you helpful instructions to take home such as stretches, heat/ice therapy, etc.

We accept cash, personal checks, Mastercard, and Visa as payment. We can also work out payment plan options if you prefer.

Finally, it’s time to schedule your next visit. Dr. Wicks will work with your schedule to make an appointment that is convenient for you.
